The Geriatric Outreach Team provides community-based case management services to Brooklyn residents age 55 and over who have an Axis I mental health diagnosis. The outreach team consists of a Social Worker, Registered Nurse, Peer Counselor, and a consulting Psychiatrist. People are linked to services in their community including outpatient mental health clinics, healthcare, home attendants, and transportation. The Program's goal is to strengthen community supports so that older people with mental illness can be safe and happy living in their community of choice.
